Treatment Support Facilitator
The primary purpose of the Treatment Support Facilitator is to provide client-centred support to people living with HIV (PLHIV) who are experiencing barriers which negatively impact their ability to commence and/or maintain HIV treatment and care.
This will require working under a case and self-management framework to build tailored action plans addressing the social determinants of health that impact treatment uptake, adherence and retention in care.
Under this case management approach, client circumstances and needs will be assessed holistically and clients will be supported to identify and overcome barriers, and develop skills to effect change in a way that is respectful of what is important to them while also achieving improved health and wellbeing outcomes.
A key aspect of supporting the achievement of client goals will be establishing and maintaining effective clinical and community access pathways.
