Case Manager – Peer Treatment Facilitator

Queensland Positive People is seeking a person living with HIV (PLHIV) who is committed to support the PLHIV community through the Peer Case Manager role. This fixed-term full time role commences in October through to June 2021 with the possibility of extension.

Aiming to address social determinant barriers to HIV care and treatment through complex case management and ultimately improve PLHIV quality of life, the Peer Treatment Facilitator will also provide educational and emotional support for PLHIV newly diagnosed and reengaging in care. The role also requires skills and experience in providing complex case management support for those experiencing challenges and issues such as mental health, finances, housing, and employment.

An exciting support and case management opportunity exists to work with Queensland Positive People a peer-based organisation which provides innovative and widely recognised support programs for people living with HIV (PLHIV) in Queensland. This fixed-term full time role commences in October through to June 2021 with the possibility of extension.

The treatment support case management role, with a strong focus on client empowerment, is based in Brisbane with possible outreach travel to other regions and provides a variety of experiences and learning opportunities for staff. The role also requires skills and experience in providing complex case management support for those experiencing challenges and issues such as mental health, finances, housing, and employment.

Responsibilities of the role are focused on supporting clients to overcome social determinant barriers impacting HIV self-management, through a client-centred case management approach. A key aspect of supporting the achievement of client goals will be establishing and maintaining effective clinical and community access pathways.

HIV/STI Test Facilitator

  • Work Type/s: Full Time, Fixed-Term
  • Location: RAPID, Fortitude Valley

The primary purpose of the HIV/STI Test Facilitator is to provide HIV/STI testing and education to people who believe they may have been at risk of contracting HIV.  This will require the delivery of time limited interventions that involve conducting appropriate test procedures, discussion of sexual health strategies and the delivery of results.

A significant requirement of this position is therefore the provision of immediate support to newly diagnosed clients and providing the necessary assistance to link the client into HIV care and treatment.  Due to the one-on-one nature of client interactions, HIV/STI Test Facilitators are expected to maintain a high-level knowledge of all aspects of HIV prevention and the HIV care continuum.

Outreach work will also be required across South East Queensland, including the Gold Coast and Metro Brisbane locations such as Sex on Premises Venues; maintaining a safe and confidential environment while working in a sexualised environment will be expected.
